Description Position at Clarvida - Tennessee SUMMARYThe Care Coordinator provides Camelot approved therapeutic interventions, case coordination and/orresource linkage to clients and other involved parties (i.e. family, DCS, Court personnel, non-relativesupports, etc.) to affect identified needed changes within the individual or family.ESSENTIAL DUTIES AND RESPONSIBILITIES•Assessing clients on intake as needed, conducting community client visitation, transporting clients,completing on-call duties, and providing Camelot approved therapeutic services to clients andfamilies including but not limited to: coping skills, behavior modification, parenting skills, and crisisintervention.•Working with assigned clients' treatment team to create, personalize, and update all Treatment Plansand other necessary documentation, and assuring that plans are consistent with Camelot policy, statelicensing regulations, and COA accreditation requirements.•Serving as a liaison and client advocate with other professionals and agencies involved in assignedclients' care.•Attending Supervision and Treatment Team meetings and making presentations on each assignedclient as required.•Supporting fellow team members to ensure the best outcomes for all Camelot clients and families.•Completing clinical documentation within established time frames and rectifying all deficits withinthe allotted time frame.•Assuring integrity and excellence by completing peer audits as assigned and contributing to thePerformance Quality Improvement Cycle, including the collection and use of data to continuouslyimprove client and program services.OTHER DUTIES•Please note this job description is not designed to cover or contain a comprehensive listing ofactivities, duties or responsibilities that are required of the employee for this job. Reasonable accommodations may bemade to enable individuals with disabilities to perform the essential functions.•The noise level in the work environment is usually moderate.TRAVEL•This position requires travel of approximately 25% of the time.SUPERVISORY RESPONSIBILITIES•None requiredREQUIRED EDUCATION AND/OR EXPERIENCE•Bachelor's degree in a Human Service discipline from an accredited four-year college or university.•1 year of relevant experience in the Mental Health field•Sensitive to other cultures and socioeconomic levels.•Has thorough knowledge of child development and behavior modification.PREFERRED EDUCATION AND EXPERIENCE•Experience working with children/adolescents in a therapeutic, community based treatmentenvironment.CERTIFICATES, LICENSES, REGISTRATIONS•None requiredQUALIFICATIONS•Language SkillsAbility to read, analyze, and interpret the most complex documents. Ability to respond effectively tothe most sensitive inquiries or complaints. Ability to write speeches and articles using original orinnovative techniques or style. Ability to make effective and persuasive speeches and presentationson controversial or complex topics to top management, public groups, and/or boards of directors.•Mathematical SkillsAbility to apply advanced mathematical concepts such as exponents, logarithms, quadratic equations,and permutations. Ability to apply mathematical operations to such tasks as frequency distribution,determination of test reliability and validity, analysis of variance, correlation techniques, samplingtheory, and factor analysis.•Reasoning AbilityAbility to define problems, collect data, establish facts, and draw valid conclusions.
