Reports to: Admissions and Discharge Coordinator

Hours: The position is Full-time, 40 hours /week. Shift may vary and will be discussed at the interview.

Candidates must be fluent in both English and Spanish (read, write, and speak).

General Statement: The Intake and Discharge Specialist supports and facilitates the intake process for new children entering our program, provides transportation services, and ensures company vehicles receive the necessary maintenance. Must have a flexible schedule to attend intakes, discharges, reunifications, and collaborate with MVM for discharge procedures.

Essential Function:

•Supports the mission, program philosophy and values of Holy Family Institute.

•Welcomes and greets new children utilizing a trauma-informed approach.

•Ensures the minor receives meals, lice treatment, clothing, and a shower during the Intake process.

•Collects necessary information and documentation for admission, including inventory of valuables and personal belongings.

•Ensures all intake supplies are available to conduct the intake.

•Coordinates and attends discharge procedures when MVM arrives to pick up a minor approved for the discharge, ensuring a smooth transition for the child leaving the program and the MVM agency transporters.

•Collaborates with the Admission and Discharge coordinators, Case managers, Case Aids, and MVM about reunifications and provides support during the process.

•Completes all necessary paperwork and documentation related to discharges and reunifications accurately and promptly. Complete Safe Reunifications & follow Policy Guidelines.

•Ensures the discharge documentation is properly completed and returned to QCR department.

•Transports minors to various locations in the USA following program needs.

•Provides an expense and cash report to Purchasing and Procurement coordinator.

•Be proactive if a rebooking becomes necessary due to cancellation or missing connections.

•Ensures minors safety and care at all times while traveling.

•Safely transport children to and from medical and dental appointments, field trips and other program activities.

•Adheres to all traffic laws and regulations during transportation.

•Reports to the Transportation Department any issues including the need for inspection, service, and repairs.

•Maintain the interior and exterior of company vehicles clean.

•Keeps records of mileage, fuel usage, and maintenance logs for each vehicle.

•Adheres to ORR, DPW, State licensing and HFI policies and procedures.

•Maintain and report standards of quality and compliance in accordance with HFI standards.

•Exhibits cultural competency and sensitivity.

•Participates in all required agency and regulatory trainings within the expected time frame.

•Works collaboratively with staff, children, families, service providers, and others.

•Promptly reports civil or criminal convictions, charges, arrests, investigations, or adjudications that may arise since staff member's last background investigation. This includes but is not limited to sexual abuse in a prison, jail, holding facility, community confinement facility, juvenile facility, other institution (as defined in 42 U.S.C. 1997), or care provider facility; conviction of engaging or attempting to engage in sexual activity facilitated by force, overt or implied threats of force, or coercion, or if the victim did not consent or was unable to consent or refuse; or who was civilly or administratively adjudicated to have engaged in such activity.

•Completes other duties as assigned by supervisor.

Requirements:

•High school diploma or equivalent; additional relevant certifications or training is a plus.

•Experience in a similar role, including intake and discharge procedures, transportation, and vehicle maintenance preferred.

•Attention to detail and accuracy in documentation.

•Understands basic vehicle's dashboard indicator display meanings, including tire pressure and fluid levels.

•Flexibility to work a variable schedule, including evenings and weekends, to accommodate intakes, discharges, and reunifications.

•Excellent written and verbal communication skills.

•Must be over 21 years of age

•Must have Act 33/34 and FBI clearances valid PA driver's license, and a clean driving record.
