Phone Triage The Resource Nurse receives calls from Hospice patients and families, assesses the needs of the patient and family, and coordinates needed care over the phone, including scheduling home visits as needed. Essential duties include, but are not limited to: 1. Perform a telephone or video assessment of patients' needs and/or concerns 2. Manage patients' pain and symptoms; obtain changes in orders as needed and update the patient's Plan of Care and medication list accordingly. 3. Provide guidance, support, and education to Hospice patients and families over the phone. 4. Assess the patient and family's need for a home visit and if so, contact the appropriate clinician to schedule the visit, and notify the patient and family of the expected visit time. 5. Communicate updates and interventions with the IDG. 6. Document all communication and/or interventions in the patient's electronic medical record. 7. Communicate with the patient's attending physician and/or the hospice medical director or designee for medical authorizations, pain and symptom management, changes to the Plan of Care, and other exchanges of information. 8. Inform family, physicians, staff, volunteers and agencies of a patient's death. 9. Relay request for a patient care volunteer, bereavement services, chaplaincy or psychosocial needs to the appropriate hospice clinician. Support for Interdisciplinary Team The Resource Nurse will provide support to the interdisciplinary team in a variety of ways included but not limited to: 1. Caseload and/or visit coverage during staff shortages or absences 2. Admission visits on an as needed basis and support to admission nurses. 3. Pain and symptom follow up calls 4. Medication refills 5. Supply / DME orders 6. Assistance with SBAR communication to medical directors for level of care change requests and prior authorization for non-formulary medications 7. New employee orientation / precepting 8. Administrative support to Clinical Services Managers 9. Compliance and quality audits 10.Notification of patient deaths to attending providers 11.Ride-along visits for annual competencies Environmental Conditions and Physical Requirements 1. Provides own transportation if needed to make a visit. 2. Successfully performs the essential duties and responsibilities of the position through the use of physical activities as described below: a. Regularly requires sitting, standing, walking, talking, use of hands, listening and observing. b. Regularly requires working on a computer or computer station for hours at a time. c. Regularly use of a telephone. d. May occasionally require reaching, stooping, bending, kneeling and lifting items 25 pounds or less.RequirementsQualifications:1. Graduate of an accredited nursing program. Current Virginia Registered Nurse license2. Minimum of three (3) years of experience as a registered nurse within last 5 years.3. Expertise or highly knowledgeable in pain management, symptom control, crisis intervention and family support4. Demonstrated critical thinking skills and excellent clinical judgment5. Experience with electronic medical records (EMR) 6. Knowledge of regulatory requirements in hospice7. Strong organization skills with sharp attention to detail 8. Excellent communication, customer service, and interpersonal skills 9. Strong critical thinking skills 10.Proof of acceptable automobile insurance and reliable transportation 11.Reliable cell phone/Home phone with local number and coverage 12.Must demonstrate basic computer skills Preferred Qualifications1. At least one (1) year of hospice, home health or palliative care experience 2. Hospice certified 3. Prior hospice triage experience 4. Knowledge of the Medicare Hospice COPs, federal, and state regulations.
