Travel Nurse Case Manager

A seasoned RN is sought by Wellspring Nurse Source for a 13-week travel nursing opportunity in Charlottesville, Virginia.
**Job Overview:**
This role involves coordinating and managing patient care to ensure they receive appropriate, cost-effective, and timely healthcare services. The ideal candidate will work across various healthcare settings to facilitate patient care transitions, advocate for patients, and collaborate with multidisciplinary teams to promote optimal outcomes.
**Key Responsibilities:
- Conduct comprehensive assessments of patients' medical, psychosocial, and financial needs to develop individualized care plans.
- Coordinate and facilitate care across multiple healthcare settings, ensuring continuity of care, appropriate utilization of resources, and adherence to treatment plans.
- Collaborate with physicians, healthcare providers, and insurance companies to authorize treatments, secure necessary services, and ensure compliance with clinical guidelines.
- Evaluate patient progress, monitor outcomes, and adjust care plans based on patients' evolving needs and goals.
- Educate patients, families, and caregivers on disease management, treatment options, and available resources to support recovery and self-care.
- Advocate for patients to ensure they receive appropriate services and that care is delivered efficiently and effectively.
**Required Skills and Qualifications:**
- Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) with valid Registered Nurse (RN) license in the state of practice.
- Case Management certification (CCM) and Basic Life Support (BLS) certification preferred.
- Previous experience in case management, care coordination, or clinical nursing, particularly in medical-surgical, home health, or specialized units.
**Estimated Salary:** $130,000 per year, based on 40 hours per week and 8-hour shifts. Tax-free stipend amount to be determined.
