Albright Care Services is a non-profit organization that helps seniors meet their health care needs in the community instead of going to a nursing home or other care facility. As a Regional Intake and Enrollment Coordinator, you will be responsible for following up with consumers who express an interest in the Albright LIFE program working across multiple locations. You will initiate the intake and enrollment process and ensure efficiency and timeliness into the program by assuming responsibility for all aspects of the admissions process. In addition, this position schedules and executes meetings with participants to ensure customer satisfaction.
Job Description
Screens intake calls, referrals and follows up on inquiries to assess qualifications for the LIFE program. Responsible for the coordination of consultations, intake evaluations and the promotion of a positive experience for all prospective participants.
Schedules and/or executes consultations with qualified leads, where the Albright LIFE program is explained in detail and if the consumer is interested in the program, will execute the intake to begin the admissions process by obtaining signatures on all required paperwork.
Facilitates the coordination of the admission by working with the state, local agencies and LIFE center staff.
Assumes responsibility for the collection of all financial and medical documents required for the Medical Assistance application and physician level of care requirement to join the program.
At time of enrollment, will coordinate or facilitate the meeting to educate the potential participant and family members about the LIFE program including the regulations that govern the services. Will obtain required signatures and process the paperwork.
To ensure enrollment opportunities are not missed, will adhere to a follow-up cadence with all qualified leads who do not enroll into the LIFE program.
Following admission into the program, will meet with all new participants to ensure customer satisfaction, with follow up with the management to share outcomes.
Qualifications
Three years of customer service or related experience in the health care field.
Working knowledge of Microsoft Word and Excel with the aptitude for learning new software and systems.
Strong time-management skills and multitasking ability.
Excellent written and verbal communication skills.
Bilingual - Fluent in English and Spanish preferred.
