What You Will Do:The Clinical Dietitian collaborates with members of the health care team to provide medical nutrition therapy through assessment of nutrient requirements for age and disease specific needs of patients in order to develop and implement the patient's nutrition plan of care. Other Responsibilities: Collects patient health data, prioritized by the patient's condition or needs, utilizing appropriate assessment techniques while involving the patient, family, and other health care providers, as indicated. Develops a plan of care, based upon identified problems, which recommends interventions to attain expected outcomes. Revises priorities, outcomes, and interventions in collaboration with the patient, family and other health care providers. Ensures systematic, on-going, data collection with relevant documentation. Analyzes the assessment data to identify immediate and long-term patient care needs. Identifies individualized expected outcomes in partnership with the patient, family and other health care providers. Continually evaluates the patient's progress toward achievement of outcomes. Communicates and documents screens, diet history, and patient's response to diet consultation as required by Clinical Practice Model (CPM). Acquires and continually enhances knowledge in current dietetic practices. Acts as a preceptor to dietetic interns at PHC facilities.
