Our cutting-edge facility provides a comprehensive spectrum of patient care services, from life-saving emergency medicine to the comforting embrace of home healthcare, with a unique range of specialized medical services, including cancer, heart health, maternity, and mental wellness.When you join the LMHS team, you become a vital part of your local community Hospital.Working at LMHS is not just a job, it is a unique opportunity to directly impact the health and well-being of your friends, family, and neighbors.You will be providing care in a place in which you are personally connected, where the impact of your work extends beyond the Hospital doors and into the heart of our community.Our commitment to diversity, equity, and inclusion ensures that every member of our community is served with respect and compassion.Join us in our mission - dedicated to patient safety, utilizing state-of-the-art technology, and with a passionate team of highly trained and compassionate individuals who strive to improve the health of the community.Position SummaryUnder supervision, and according to state and federal law covering the practice of pharmacy, establishes policies and procedures; compounds and dispenses medications and other pharmaceuticals supplies.Provides a pharmaceutical service and education for a patient population that includes ages ranging from newborn through geriatric.Works with multi-disciplinary health care team across various inpatient practice settings.ResponsibilitiesProvide de-centralized clinical pharmacy services for diverse patient population.Monitor patient medications to determine possible medication therapy problems.Verify inpatient physician orders and follow protocols/procedures.Dispense medications and manage technician refilling of automated dispensing cabinets.Supervise technician and/or prepare intravenous admixture solutions according to physician orders.Supervise pharmacy technicians on duty for accuracy of work.Provide clinical guidance to multi-disciplinary staff pertaining to medications as needed for acute care patients.Respond to Code Blues, Traumas, STEMI's, and Rapid Sequence Intubations in the ED and throughout the hospital. Provide drug information as requested to physicians, nurses, and patients.RequirementsPharm D or Bachelor's degree in pharmacy.Licensure as a Pharmacist in the State of Ohio.3-5 years of acute care experience preferred.ACLS certification required (opportunity to obtain this post-employment will be provided if needed).PALS certification preferred (opportunity to obtain this post-employment will be provided if needed).Strong analytical ability to determine drug appropriateness and evaluate drug instructions.Ability to provide care for a neo-natal, pediatric, adolescent, adult and geriatric patient population.Specific disease state and/or Board of Pharmacy Specialties certification preferred (opportunity to obtain this post-employment will be provided if needed).Opportunity to cross-train to other pharmaceutical specialties (EG Oncology, Emergency Medicine, Critical Care Medicine, Medication Therapy Management) will be provided as department needs and pharmacist aptitude allow.
