Description

The Older Youth Wellness Coordinator works with identified youth in the foster care system, providing support and guidance, helping to navigate the challenges of foster care. Responsibilities include establishing meaningful connections with the youth, offering emotional and educational support, and assisting in the development of essential life skills. The role requires a compassionate and understanding approach to address the unique needs of older youth in foster care, promoting their well-being, stability, and successful transition into adulthood. Effective communication, advocacy, and collaboration with other professionals in the foster care network are key aspects of this impactful position

What to expect on a given day as an Older Youth Wellness Coordinator:
  • You will track and monitor all identified older youth, including location, education, medical and psychiatric appointments in the field, to assure appropriateness of care.
  • You will complete deep dive evaluations on identified youth.
  • You will provide emotional and educational guidance to enhance their well-being.
  • You will collaborate with community resources to ensure access to housing, healthcare, and educational opportunities.
  • You will follow up to ensure medical appointments and paperwork associated with appointments have occurred.
  • You will assist case management staff to identify and locate prior/current established positive supports for the youth.
  • You will track medical, dental and eye examinations for all foster youth every six months during their tenure in foster care and provide reports every three months on the status of all examinations.
  • You will work closely with case managers, social workers, and other professionals to create a comprehensive support network.
  • You will assist Foster Care case management staff with identification of specialized referrals as necessary for foster youth.
  • You will track youth that have run from their placements, and facilitate communication with detectives, and the National Center for Missing and Endangered Children.
  • You will assist with transport for children and youth to appointments and outings.
  • You will collaborate with educational institutions to support academic achievement, and vocational training for a successful transition to independence.
You would be a good fit for this position if you:
  • Have a bachelor's degree in social work required.
  • Have a minimum two years of related experience.
  • Have experience working in community based social services.
  • Have successfully completed the Children's Division Basic Training (or equivalent) for provision of case management services in the State of Missouri.
  • Have experience in serving high needs populations.
  • Have excellent written and oral communication skills.
  • Can handle confidential information, administrative details, work independently, prioritize work, and meet deadlines.
  • Can be flexible and adapt to changing needs and time pressures.
  • Can engage difficult to reach, traumatized, or disengaged individuals and families.
