Description
Case Manager provides compassionate, professional trauma-informed, client-focused, case management services to individuals, families, or groups receiving services within the Case Management Pilot Program at Lutheran Social Services of the National Capital Area in Washington DC and Northern, Virginia. The ideal candidate will have extensive culturally appropriate case management experience working with asylum-seekers, refugees, immigrants, victims of human trafficking, and/or victims of domestic violence. The candidate must have a commitment to the mission of LSSNCA, as well as the diverse refugee and immigrant populations which we have served for over 100 years in the National Capital Area.
ESSENTIAL FUNCTIONS
- Realize clients' care by assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress.
- Admit new clients by reviewing records, conducting intakes and orientations. Coordinates and provides care that is safe, timely, effective, efficient, equitable, and client centered.
- Determine clients' requirements through intake interviews, assessing the need for therapeutic medical, psycho-social, and psychiatric evaluations.
- Develops culturally appropriate and responsive service plans, and provides outpatient services including orientations, classes, screenings, assessments, and referrals using evidenced-based and trauma-informed practices.
- Establish treatment programs by setting schedules, coordinating services, and arranging resources, including transportation and escort.
- Monitor cases by verifying clients' attendance, observing, and evaluating treatments and responses.
- Works cooperatively with service coordination team, including attorneys, service providers, community partners, and government entities.
- Leads clinical documentation, i.e. case notes, patient assessments, service/treatment plans, and other forms/records as required in a timely manner. Maintains detailed case files and other documentation in 100% compliance with the CMPP program standards.
- Develops and review case plans in consultation with clients and service coordination team and perform follow-ups on quantity and quality of services provided. Communicate clients' progress through weekly interdisciplinary meetings, disseminate results, and identify treatment influences.
- Provides short-term crisis intervention to address immediate mental health emergencies, stabilize the individual in crisis, create and implement a safe, appropriate plan.
- Prepare clients' discharge by reviewing and amplifying discharge plans, coordinating post-discharge requirements, and providing resources.
- Improve staff competence by providing educational resources, balancing work requirements with learning opportunities, and evaluating the application of learning to changes in treatment results.
- Assists in the collection, analysis and reporting of program data to demonstrate the progress or impediments of reaching program goals and targets. Participates in and collaborates in recurring data quality assessments.
- Update job knowledge by participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations.
- Meets or exceeds program productivity expectations on a regular and sustained basis.

- Bachelor's degree in preferably human services
- Minimum of 2 - 4 years of relevant experience working with vulnerable or migrant populations.
- Knowledge and experience in trauma-informed case management practices.
- Strong organizational skills and ability to manage multiple projects simultaneously with attention to detail.
- Excellent interpersonal skills, including good communication skills, "active" listening skills, and attitude.
- Strong analytical and data management skills.
- Proficient in data collection, electronic medical record database and Microsoft Office products.
- Maintain patient, team member and employer confidentiality, compliance with all HIPAA regulations.
- Must pass required physical, TB test, and drug screening.
- Must pass required child registry clearances for the District of Columbia, Maryland, and Virginia.
- Must be eligible for employment in the U.S.
- Valid driver's license, access to an automobile, insurance, and willingness to drive to off-site locations.
- Spanish fluency preferred but not required.
