The Intake Specialist is one of the initial points of contact to ensure members have a positive and successful entry into our facility. This position is accountable for reviewing eligibility and program expectations prior to members meeting with their assigned clinical workers. This position requires a thorough understanding of the company's philosophy of treatment, admission processes, and program criteria. A basic knowledge of substance abuse and dual diagnosis is also required. This role is responsible for ensuring that clinical and administrative intake services are provided for clients in partnership with the clinical team and other cross functional departments.Duties and Responsibilities:Initiates the lobby packet paperwork and consent/admission formsCompletes the intake assessment, effectively and correctly identifying and documenting appropriate diagnosesCompletes the individualized treatment plan, based on the treatment goals identifies through the completed assessmentExplains the program to the member and ensures the member comprehends informed consent. This is to include:State specific requirements, program policies, and the need to report any suspected child abuse and neglect forms of abuseVerification of member's involvement in collaborative treatment options, to include their goals for treatment - Member signature should be included for acknowledgement verificationImportance of treatment complianceResources for comorbidities, dual diagnosis if neededMembers' rights and responsibilitiesAll program options available to member, his/her rights and responsibilities in the programGrievance and appeals proceduresWays in which input is given regarding the quality of care, achievement of outcomes, and satisfaction of the person servedThe organization's services and activities; expectations; hours of operations; access to after-hour services; code of ethics; and confidentiality policyProgram's policies regarding use of tobacco products, illicit or licit drugs brought into the program, and weapons brought into the programIdentify the person responsible for coordinating services for the memberProvide a member handbook explaining the program, expectations, rules, rights, and explanation of behaviors or attitudes that may lead to the loss of rights or privileges.Education regarding advance directivesIdentify the purpose of the assessment and the processVoluntary participation in the programCompliance with Federal Confidentiality RegulationsAll new admits and readmits will be oriented, if possible, on the first day of treatment - In all cases, orientation must take place within seven (7) working days of admission to treatmentCompletes hand-off to the clinical and cross-functional member support departments once all intake tasks are completed successfully for the memberEffectively meets any/all outlined Key Performance Indicators (KPIs), ensuring the highest quality and level of member care and engagement is providedReceives and actively engages in clinical supervision and oversight each week/month based on BHPP/BHT designation requirements, including completion of all continuing education requirementsIn addition, all other duties as assignedAt New Freedom, we restore lives and reintegrate communities through intentional guidance by providing our members with a sense of identity, purpose, and hope.
