Now accepting applications for a Per Diem Social Worker with SNF experience. This position coordinates admissions/discharges of residents in healthcare and maintains appropriate contact with hospitals, doctors, and outside agencies to facilitate admissions.

POSITION SUMMARY : Coordinates or performs the necessary administrative functions regarding the admissions/discharges of residents in healthcare. Assist in the organization and planning of the Social Services provided for residents of this community.

ESSENTIAL FUNCTIONS :

? Assists with the responsibilities and functions delineated in the discharge plan.

? Ensures that the administrative and clerical functions related to the admission and/or discharge of residents is carried out to completion by coordinating efforts with the Healthcare Team Members.

? Maintains appropriate contact with hospitals, doctors and outside agencies to facilitate admissions.

? Coordinates all forms needed for admissions.

? Maintains state required resident roster.

? Coordinates financial updates for private pay residents.

? Completes appropriate section of the MDS.

? Participate and attend all in-service training and education programs as scheduled.

? Other duties as assigned.
