The Director of Rehabilitation will seek to return the resident to the optimal physical function and facilitate achievement of resident, Physician, and therapy goals. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Rehabilitation Policies and Procedures.Qualifications/ Responsibilities:Must possess hold a valid and current license as a therapist or therapist assistant granted from the state of Virginia.Participate in community planning related to the interests of the facility and the services and needs of the resident and family.Participate in discharge planning, development and implementation of social care plans and resident assessments.Interview resident/families to obtain social historyRefer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident.Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.Previous experience required, experience in PDPM preferred.Shift & Wage:Full-Time Opportunity AvailableShifts Needed: VariesCompetitive SalariesSign-on Bonus: $10,000 About Us:We strive to be the leader in the long-term care industry operating in 6 different states.
