Position Summary:

Patient Navigator will serve as a liaison between Homeless Service Providers & Saban Community Clinic in order to improve the coordination of patient care for people experiencing homelessness. Our Patient Navigator will work closely with internal & external teams. This individual will support Case Managers and Clinicians to assist their most acute clients in accessing regular clinical (medical, dental & behavioral health) care at Saban Community Clinic.

Duties and Responsibilities:
  • Will be required to be the primary driver for our Mobile Van, driving the van to a different site every day and then driving it back to it's parking site each night.
  • Serve the Saban mobile medical & dental clinic as the Patient Navigator
  • Build relationships with program staff in order to coordinate care for unhoused patients in various settings
  • Serve as a liaison between patients and healthcare providers - both at the Emergency Room and shelters/programs in partnership with Saban Clinic
  • Correspond directly with care providers to develop care plans for patients & assist with specialty medical & dental care referrals; Care providers include, but are not limited to: Saban providers, staff at partner organizations, & staff at external organizatoins as appropriate
  • Link patients to a medical & dental home as well as community resources and services to support their overall wellness and recovery
  • Work in collaboration with Program Manager to create ongoing communication between multiple systems
  • Facilitate the flow of information regarding updates and changes between the mobile van program and Saban Community Clinic teams
  • Meet with patients to obtain information about their financial circumstances and informs them of the availability and eligibility requirements of various health insurances/programs (e.g., Medi-Cal, My Health LA and others)
  • Assist patients to apply for all possible health insurances/programs for which they qualify, providing forms and assistance as needed; review documentation and follow up to ensure patients apply; resolves related problems
  • Obtain proof of insurance (if any), Medi-Cal/HMO coverage or other program that will reimburse for medical expenses at the Clinic
  • May provide clients with transportation assistance.
  • Work with SCC staff to facilitate patients into a medical, dental, or behavioral health in-clinic appointments
  • Conduct community outreach at mobile van housing sites
  • Track and regularly update data sheets pertaining to program outcome measures (as applicable)
  • Represent Saban Community Clinic in all matters of healthcare delivery and community outreach
  • Attend and actively participate in internal and external meetings, workshops, and trainings. Other relevant tasks as assigned.
Position Requirements:
  • Passion for working with acute, disabled and vulnerable individuals, including individuals experiencing homelessness
  • Experience navigating the healthcare system for high acuity clients
  • Detail oriented with strong time management, organizational, written, verbal, and interpersonal skills
  • Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers
  • Skill in non-violent crisis intervention
  • Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Office programs
  • Able to pass a background check
  • Current California driver's license and State-required insurance when using personal vehicle on Clinic business
  • A driving record acceptable to the organization and/or its insurance carrier so the candidate can drive the Mobile Van.
