The community culture is fitting for outdoor lovers, bikers, hikers, or those just simply soaking in the panoramic views.ESSENTIAL FUNCTIONS:1.Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.2.Communicate necessary information to others as appropriate.3.Evaluate and treat acute care and swing bed patients as prescribed by a licensed physician, with a wide spectrum of diagnoses.4.Supervise and direct occupational therapy assistants, occupational therapy students and rehab aides.5.Identify and initiate appropriate physical therapy interventions.6.Assess and document the effectiveness of treatment and modify the treatment plan to achieve goals.7.Assess educational needs of the patient, family or caregiver and provide education to meet those needs.8.Provide care with the consideration of the physician's orders and established plan of care.9.Communicate with patient, family, caregivers and members of the healthcare team to promote maximum benefit of care.10.Work cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.11.Document objective measures of patient status, within the limits of his/her licensure and ability, as appropriate, in providing care and to assist in assessing the effectiveness of care.12.Communicate the goals and treatment plans of shared patients with the Physical Therapist Assistant, while delegating appropriately, to promote maximum benefits from therapy.13.Implement education plan as outlined by the evaluating therapist, determine additional educational needs of the patient, family, care giver or significant other as they arise and in collaboration with the evaluating therapist.14.Follow plan for discharge and discuss with evaluating therapist any additional discharge concerns, which arise during care.15.Complete all documentation required to support the services provided and meet organizational, clinical and reimbursement requirements.16.Maintain positive relations withal customers and family members.17.Ensure that patient charges are accurate and entered within 24 hours of service.18.Complete required documentation, including progress notes, daily notes, discharge notes, and insurance forms within 24 hours of service. Complete patient evaluations within 24 hours of service.19.Contribute to the cost effectiveness of services and programs provided by the department.20.Participate in personal performance improvement and program development activities including administrative staff meetings.21.Communicate observations and suggestions related to opportunities for improvement to supervisor.22.Orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual as requested.23.Assist with administrative duties, including but not limited to answering phones, billing, photocopying and clean up, as necessary for the overall department function when necessary.24.Facilitate referral base by effectively communicating with referral source through personal visits, phone conversations or written messages.25.Perform Physical Therapy services off campus as directed by Rehabilitation Services Director.
