RN / Registered Nurse

Job Responsibilities:

Perform health assessments for patients in their home to assess a patient’s health care status and needs at start of care and then every 6 months and as necessary

Initiate the plan of care and makes necessary revisions as patient status and needs change

Document patient history, symptoms, assessments, tests, and care according to guidelines

Supervise paraprofessional staff on patient plan of care

Be a Care Manager for your patients

Educate patients and their families on proper home health care strategies and procedures

Reporting all issues and concerns to the Director of Patient Services

Complete documentation accurately and submit timely to agency

Provide the highest quality care in accordance with DOH regulations

RN / Registered Nurse

Requirements:

Must possess a current, valid NYS RN license and Registration

Government Issued ID, Social Security Card & Work Authorization

2 Reference Letters

Recent Physical

TB Screening (Quantiferon required)

Rubella & Rubeola titer lab work

Excellent communication skills

Strong critical thinking skills

Able to function independently with minimum supervision

Dedicated to providing the best patient care possible

Ability to travel to patient’s homes

Location:

Visits to be completed in 5 boroughs and Nassau
