Case Manager/Care Coordinator, BHMA (FT) 8a-5p, Care Management Job ID: 004D6I INTEGRATED SOLUTIONS HLTH NTWK Nursing - Care Management Ambulatory Full-time (scheduled 72 hrs or more per pay period) - DayJob Description SCOPE OF POSITIONThe Case Manager/Care Coordinator provides transition of care, case management, coordination of care, and disease management support for high risk patients and patients with chronic diseases. In addition, this individual will assist patients by making appointments for needed follow up. The Manager will transition and coordinate care using collaborative relationships with providers, office coworkers, hospital coworkers, and families. The Manager will act as a health coach with patients and assist them through coordination of care, wellness through prevention, support, encouragement and education.The Manager will be responsible for documenting goals, plans, orders, educational services, educational interventions, and outcomes. The Manager will provide education and training to participants and caregivers for self-management of a chronic disease. The Manager will develop educational plans and goals in concert with the provider and health care team directed toward helping each individual achieve self-management of their health; as well as addressing barriers or vulnerabilities that would hinder optimum results with the care plan. The Manager will provide review of medical records and additional means to evaluate quality of care and compliance with practice guidelines and national evidence-based health criteria. The Manager is responsible for review of criteria pertinent to adopted quality criteria at provider level.REPORTING RELATIONSHIPSee Table of Organization.EDUCATION AND EXPERIENCEBachelor's degree in Nursing , Social Work, Counseling, or Marriage/Family Counseling preferred. Five years clinical experience with chronic disease management, substance use/abuse or mental health challenges.Valid and active RN, Social Work, or Therapist licensure in appropriate state preferredCPR requiredRequirements No additional requirements from any stated in the above description.
