We provide home health physical therapy, occupational therapy, and skilled nursing services. We are CHAP accredited and a certified Medicare and Medicaid provider. Job Summary:

Were actively seeking reliable and dedicated Occupational Therapists (OT) with home health or geriatric experience to provide top-notch hands-on care. Employees must be able to communicate with MDs and collaborate with the patients interdisciplinary team. New employees will be required to attend general orientation, transfer training, and documentation system training as well as complete online virtual education series quarterly. Ideal candidate must be agreeable to perform evaluations and treatments and complete 5+ points per week. Occupational Therapists will be expected to perform outcome measures on patients to allow for objective goals, progress tracking, and company metrics. Responsibilities included but not limited to:

  • Initiating Occupational Therapy Evaluation and Plan of Care: Occupational Therapists will perform Occupational Therapy evaluations and establish the plans of care in addition to providing guidance to other personnel or family/caregivers involved in the patients care as necessary.
  • · Outcome Measures:Occupational Therapists will be responsible for performing specific outcome measures with all occupational therapy evaluations, 30-day re-assessments, and discharges for objective goal making, progress tracking, and data collection.
  • Delivering Occupational Therapy Treatments: Occupational therapists will be expected to complete occupational therapy treatments to patients when it is clinically required and/or when assigned by the office.
  • 30-Day Reassessments: Occupational Therapists will create and submit 30-day reassessments detailing the patients progress and response to treatment.
  • Supervising Occupational Therapy Assistants: Occupational Therapists are responsible for overseeing the work of Occupational Therapy Assistants in accordance with established policies and state regulations to maintain quality of care. The occupational Therapist will be responsible for completing the supervisory visits in addition to the 30-day reassessments when an OTA is involved in the patient’s care.
  • Participating in Discharge Planning: Occupational Therapist will be responsible for completing the Occupational Therapy discharge summary for expected and unexpected discharge which includes collaborating with the healthcare team and caregivers to ensure a smooth discharge.
  • MD orders and communication: Occupational Therapists will be responsible for creating a plan of care and following a plan of care with adherence to MD orders. Occupational Therapists will communicate with MD regarding patient change of status, abnormal vitals, or medical concerns and communicate results with the office independently.
  • Ensuring Legal and Regulatory Compliance: Occupational Therapists are responsible for ensuring adherence to all state and federal laws and regulations governing occupational therapy practices to maintain legal and ethical standards as well as maintain an active Occupational Therapy license in the state of Virginia.
  • Timely Documenting: Occupational Therapists are responsible for performing high-quality documentation, and corrections, timely.
  • Communication: Occupational Therapists will be expected to be able to communicate with their supervisor and the office in a timely manner when requested.

  • · Communication Skills: The ability to communicate effectively, both verbal and written in English, is required for this role.
  • · Driving Ability and Home Accessibility: OTs must hold a valid drivers license and be capable of accessing clients residences, which may not always be wheelchair accessible.
  • Physical Capability: Adequate hearing, eyesight, and physical agility are imperative for performing occupational assessments and delivering client care. The role entails a variety of physical tasks including walking, sitting, stooping, and standing. Occasionally, OTs may need to bend, stoop, reach, and handle patient weights of up to 300 pounds, as well as lift and/or carry objects weighing up to 50 pounds.
  • Medical Equipment Familiarity: OTs should be familiar with using medical equipment such as blood pressure cuffs, thermometers, and stethoscopes. Access to hand-washing materials is necessary to maintain cleanliness and infection control standards.
  • Access to Patient Medical Records: OTs have access to all patient medical records, which may be discussed with the Clinical Manager, Administrator, Executive Director, and Quality Assurance department staff.

Qualifications:

  • OT Degree: Successful completion of a degree program accredited by a recognized institution in the field of Occupational Therapy.
  • Experience: Minimum of one year of experience working as an Occupational Therapist.
  • Home Health Experience: Six months to one year of experience in a home health setting is preferred.
  • Geriatrics Experience: Experience in geriatric care is preferred, as the main demographic served in home health settings is often elderly individuals.
  • License: Possession of a valid and unencumbered license to practice y in the state of Occupational Therapy Virginia.
  • Organizational and Communication Skills: Strong organizational and communication skills to effectively interact with clients and healthcare team members.
  • Flexibility: Ability to work flexible hours to accommodate clients needs and scheduling demands.
  • Background Check: Completion of a background check to ensure compliance with regulatory and organizational requirements.
  • CPR Certification: Must possess a current CPR certification from an accredited provider. Online certifications are not accepted.
  • Drivers License and Transportation: Must have a valid drivers license and reliable transportation with current automobile insurance.
  • COVID-19 Vaccination: Compliance with Jobeka Health Care Services LLCs policy requiring full vaccination against COVID-19. This includes receiving a minimum of one dose of a one-dose vaccine or two doses of a two-dose vaccine.
