Position: Occupational Therapist - Home Health Care


Location: Richmond, Virginia

Responsibilities:

  • Conduct comprehensive assessments to evaluate clients' functional abilities and limitations.
  • Develop individualized treatment plans to help clients achieve their goals for independent living and optimal functioning.
  • Provide skilled occupational therapy services to clients in their homes, including therapeutic exercises, adaptive equipment training, and environmental modifications.
  • Monitor clients' progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Collaborate with interdisciplinary healthcare team members to coordinate care and maximize clients' independence and quality of life.

Requirements:

  • Valid license to practice Occupational Therapy in the state of Virginia.
  • Previous experience in home health care or geriatric care preferred.
  • Strong clinical assessment, communication, and interpersonal skills.
  • Compassionate and client-centered approach to care.
  • Age Friendly Care 
  • CHAPS accreditation knowledge or experience is a plus.
