General Description:

This grant-funded position is responsible for facilitating, coordinating, and tracking the school healthcare referral network and processes in Hallandale Beach, FL within three Community Partnership Schools at Gulfstream Early Learning Center, Gulfstream Academy of Hallandale Beach K-8, and Hallandale Magnet High School.
Primary Job Responsibilities:
Facilitate relationships with local healthcare providers to build and maintain on campus health services for students, staff, and familiesat Gulfstream Early Learning Center, Gulfstream Academy of Hallandale Beach K-8, and Hallandale Magnet High School.
Coordinate and track theCommunity Partnership School (CPS) healthcare referral network and processes in Hallandale Beach.
Serve as the point of contact between CPS administration, school nurse, medical clinic, behavioral health department, and other health-related agencies/departments providing services.
Serve as entry point to the Community Partnership School for students and parents.
Secondary Job Duties:
Establish referral network/linkages with outside agencies.
Match all medical clinic referrals, including health, immunization, dental, vision, and behavioral health referrals.
Track and establish follow up procedures for CPS referrals made in and out of school.
Outreach through phone call or written communication to identify individual needs and program eligibility.
Participate in school Intervention Team meetings to provide information on behavioral interventions and relevant referrals as needed.
Coordinate holistic health and wellness events for students, staff & community, including the annual Peace & Health Festival each February.
Maintain accurate and complete parent and community member participation and engagement data.
Utilize data management system for proper documentation.
Implement and oversee eligibility, billing, and other reimbursement procedures.
Submit all documentation in a timely manner (data, trackers, etc.)
Contribute to a positive, engaging work environment.
Develop a strong knowledge base and stay current on job-related issues and trends.
Participate actively in departmental meetings, training, and education, as well as the quality improvement process.
Assist with training other team members and providing back up when necessary.
Supported by Community Partnership School Manager, liaison with school administrative personnel to align system goals, program objects and community services; serve as a core member of the Community Partnership School Operations Team.
Build rapport with teachers, staff, parents, and community members to be an advocate for students and community needs.
Other Job Duties:
Performs other duties and assumes other responsibilities as requested by supervisor.
Present the Community Partnership School model to multiple stakeholders, including students, parents, and community leaders.
Prepare written materials for the purposes of documenting activities, providing written reference and/or conveying information.
Participate in the quality improvement process.
Collaborate in fundraising and grant writing.
Possible arrange for transportation of clients.
As needed, work flexible hours including evenings and weekends,
Due to the leadership role United Way is called upon to play in the community, particularly during times of crisis, it is the expectation that all United Way staff will be fully engaged in the organization's crisis plan and response efforts.

Actively participate in the COA accreditation process.

Education, Skills and Experience Requirements:
Graduation from an accredited college or university with a bachelors degree in a Human Services, Education, Social Work, Psychology, Public Health, Counseling, Nursing or related field and one to two years of combined experience in community outreach

graduation from an accredited college or university with an associate degree in a Human Services/Social Services related field and three to four years of combined experience in community outreach.
OR
high school diploma or GED equivalent and five years of relevant experience.
Be energetic, passionate, and adaptable with a deep commitment to social service, empathy for children and families and a positive approach to embracing and managing change.
Experience with building community relationships, convening workshops and working with families on educational or community issues.
Demonstrate understanding of high-needs communities and knowledge of community organizing principles.
Experience working with data to inform practice.
Proficient skills working with computers and software programs.
Demonstrated ability to build and maintain effective working relationships with key stakeholders, in the school and the healthcare community.
Excellent organizational, verbal, and written communication skills (English required; Spanish, Haitian Creole, Russian, or Ukrainian highly desirable and preferred).

Environment & Physical Demands: The environment is climate controlled (i.e., heated and air-conditioned). Lighting is adequate through overhead lighting as well as windows and blinds. Physical demands include sitting and/or standing for extended periods of time, bending, lifting approximately 25 pounds, regular use of the telephone, copier, fax, and computer.
