Clinical Support LPN
*This is an office based role
Full Time
Days (7:30am - 4:30pm or 8am - 5pm)
No weekends or holidays
JOB SUMMARY
Works in collaboration with Clinical Support RNs to triage calls from the 24-hour phone number and serves as a resource to the home hospice clinical staff in the field.
ESSENTIAL FUNCTIONS
Provides immediate response to all incoming clinical calls.
Triages calls assessing pain and symptoms, physical needs, and disease progression.
Provides instruction to the caller or collaborates with the RN for necessary follow-up, prioritizing those with uncontrolled pain/symptoms and imminent death.
Assures interdisciplinary team visit for new symptom, death, or crisis.
Follows up after hour calls/visits/client issues and concerns as requested.
Demonstrates understanding and application of palliative care protocols for symptom management, in collaboration with RN.
Supports the initiation of system management orders through timely notification to contracted pharmacy.
Obtains refills, renewals, new and revised orders for medication and treatments in collaboration with the RN.
Follows up with provider offices for outstanding or unclear orders.
Documents in an accurate and concise manner on the day the service was provided, supporting the limited prognosis, the level of care, changes to the plan of care, disease progression and care needs.
Maintains electronic patient records.
Communicates effectively with organizational staff, physicians, patients, families, nursing home staff, outside contractors, community agencies.
Informs other disciplines and volunteers of changes in condition, goals, and plan of care.
Provides instruction to patients and caregivers regarding diagnosis, care, treatments, medications, and end of life expectations.
Understands reimbursement and payer source implications to the plan of care.
Assists with the orientation of new staff
QUALIFICATIONS
Must have current Pennsylvania nursing license.
One year of recent medical/surgical nursing experience as a licensed practical nurse preferred
