The Preferred Communities (PC) Case Manager will provide outreach, case management and referral supportive services to Office of Refugee Resettlement (ORR) eligible population including those that are especially vulnerable. This may include specialized support for Afghan and Ukrainian paroles. The PC Case Manager ensures compliance with PC grant requirements. DUTIES

  • Develop and lead an outreach plan, engage in public speaking, and ensure those seeking support are connected to services.
  • Screen and enroll eligible families and individuals to the appropriate PC case management services per screening result.
  • Develop individualized, self-sufficiency plans for each client enrolled in the PC program.
  • Provide client-centered, trauma-informed culturally and linguistically appropriate assistance to ORR eligible clients enrolled in the program through Intensive Case Management or Gap Services for 3-12 months per specific program guidelines.
  • Provide Intensive Case Management services, as applicable, to especially vulnerable individuals enrolled in the program including the provision of cultural adjustment support, assistance in accessing appropriate medical and mental health support, assistance in accessing or maintaining social benefits such as Medicaid and SSI, and needed follow up services for the duration of the program period.
  • Coordinate and communicate with resettlement and employment team in creating long-term self-sufficiency and integration plans.
  • Provide ongoing education about health-related issues, including insurance coverage, identifying, and selecting physicians who provide language appropriate services, and basic public health counseling.
  • Provide cross-cultural and enhanced orientation to ORR eligible population as needed.
  • Cultivate new relationships with service providers and referral partners including those that enhance support services including but not limited to women empowerment programs.
  • Adopt standard operating procedures to ensure quality support.
  • Assess financial need and help families with applications to programs and resources including, assistance in enrolling in applicable benefits to ensure self-sufficiency.
  • Advise management team of any issues arising from sponsors and parolees.
  • Ensure ORR eligible population are connected to immigration legal services, as applicable.
  • Maintain frequent contact with assigned clients including home visits.
  • Coordinate and collaborate with key stakeholders and service providers to ensure that program participants are linked and offered services in a timely manner without duplication of efforts.
  • Maintain up-to-date documentation in client's case file and online database to ensure services are properly documented throughout the client's service period.
  • Submit timely programmatic reports.
  • In coordination with the Volunteer Coordinator, refer clients to one-on-one volunteer mentoring as needed.
  • In consultation with the supervisor, ensure close collaboration and coordination with the State Refugee Coordinator's office and other service providers regarding efforts to support ORR eligible population.
  • Ensure that clients are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary.
  • Participate in PC program network meetings and training.
  • Other duties as assigned.

QUALIFICATIONS

  • Bachelor's degree in a Social Work or behavioral/human services field or two years of experience providing case management and/or social services preferred.
  • Knowledge and understanding of refugee resettlement systems, or other relevant cross cultural experiences a plus.
  • Ability to maintain a pleasant, customer-centered demeanor and professional image under a variety of challenging situations.
  • Fluency in one of the ORR eligible population languages is highly preferred including Pashto, Dari, Ukrainian, Russian, Spanish, Creole.
  • Proficient in the use of MS Word, MS Access, MS Excel and database programs.
  • Ability to work efficiently in a fast-paced environment, troubleshoot, and follow projects through with attention to detail.
  • Ability to work as a team member and independently, with high level of self-motivation.
  • Exceptional interpersonal skills, including keen listening ability, relationship building

and community outreach.

  • Desire to connect with people from diverse backgrounds.
  • Must have reliable transportation, clean driving record, and valid insurance and be able to travel locally as required.
  • Cross-cultural or international experience preferred.
