Job Title: Health services Rep/Transporter

Reports To: Clinical Supervisor

FLSA Status: Non-Exempt

Department: VDR Clinic

Summary: Responsible for support and transport for clinical appointments, medical and dental appointments for facility residents. Coordinate and follow up on client appointments.

Responsibilities and Duties:
Coordinates out-patient clinical appointment; follow up on appointments including the special medical procedures with conservators if needed.
Provides support and transport for medical and dental appointments for facility residents. Models flexibility and ability to handle urgent/emergency appointments as needed.
Must have current personal automobile insurance coverage. Must have a clean DMV pull report. Must be a minimum of 25 years of age. Must give a copy of proof of coverage with copy of the insurance card.
May provide direct support and guidance to clients to make insurance changes as needed.
Prepares necessary documents such as referral forms, current medication orders, insurance verification forms, etc. for the outpatient clinics.
Provides general transportation in agency vehicles including but not limited to the transportation of clients to off-site outings and appointments, for program supplies and on agency business.
May provide assistance and supports to clients in all areas of daily living, working individually and in groups.
Observes, performs and documents narrative entries pertinent to the health of resident in the daily log.
Follows established policies and procedures in a timely manner and assures paperwork/reports are completed on time and as directed.
Monitors residents behavioral changes, reinforces positive appropriate behavior, offers counseling and support to residents in need, provides documentation accordingly and reports observations to the supervisor; monitors for signs and symptoms of mental illness.
Identifies and relays key clinical information between medical professionals on a case by case basis for non-routine items.
Supports other program staff by encouraging all clients to attend activities, groups, and scheduled appointments.
Performs first aid and CPR in emergency situations as needed.
Reports abuse as mandated.

Education/Experience: High school diploma or general education degree (GED) and one to two years related experience and/or training preferred; or equivalent combination of education and experience.

Language Ability: Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Must have good people skills and the good telephone communication skills.

Math Ability: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.

Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems in standardized situations and emergency situations.

Computer Skills: To perform this job successfully, an individual should have knowledge of Microsoft Outlook and calendaring; Microsoft Word; Microsoft Excel; and basic online browsing/usage.

Certificates and Licenses:
Medication Room Training completion within 30 days
CPR and First Aid Training
Must be 25 years of age, hold a valid drivers license, current auto insurance, and clean DMV record

Equipment: Fax machine; PC Computer; Multi-Line Phone; Car/Van

Work Environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee is occasionally exposed to toxic or caustic chemicals and outdoor weather conditions. The noise level in the work environment is usually moderate.

Special Requirements: Must be able to meet and receive a criminal record clearance as required by CCR Title XXII, licensing regulations. This includes FBI and DOJ clearance. A health screening, by or under the direction of a physician must have been performed not more than one year prior to employment or within seven(7) days after employment. Health screening must include TB clearance not more than one year prior to employment or within seven (7) days after employment. Must have current standard first aid and CPR card from an accredited institution. Must be physically able to assist clients and /or staff in the evacuation of program facility in emergency situations. Must be able to administer first aid and CPR in an emergency situation.
