Woodland Manor Nursing and Rehab is seeking a MDS Registered Nurse Assessment Coordinator (RNAC) to join our skilled nursing facility in Elkhart, IN. This vital role involves coordinating and completing resident assessments in accordance with state requirements and facility policies.
Position Details
  • Location: Elkhart, IN
  • Schedule: Full-Time, Monday - Friday
  • Salary: Around $80,000/year, depending on experience (DOE)
Key Responsibilities
  • Conduct and coordinate resident assessments on a routine schedule or as needed for condition changes, hospital returns, or other factors.
  • Accurately observe, assess, and communicate condition changes to appropriate personnel.
  • Ensure completion of assessments and CAAs by interdisciplinary team members, such as Social Services, Activities, and Dietary, and obtain necessary signatures.
  • Complete additional MDS sections, quarterlies, CAAs, and care plans as needed.
  • Develop and update care plans based on resident needs identified in assessments.
  • Prepare and distribute MDS schedules to the interdisciplinary team.
  • Transmit MDS data to the state weekly (or as required) and maintain accurate records, including transmittal logs.
Qualifications
  • Licensure: Must be a state-licensed Registered Nurse (RN).
  • Experience: At least one year of experience in long-term care as an MDS Assessment Coordinator.
