We are seeking a skilled Nurse Practitioner to join our team at Matrix Medical Network.

The ideal candidate will have experience in conducting adult/geriatric assessments, medical history, diagnosis, and treatment, as well as health education and case management.

This is a PRN/Part Time opportunity with flexible scheduling, including days, evenings, nights, and weekends. The role will involve conducting home risk assessments, working collaboratively with physicians, case managers, and other healthcare professionals.

Key Responsibilities:

  • Conduct comprehensive health assessments, including medical history, diagnosis, and treatment.
  • Develop and implement individualized care plans to improve patient outcomes.
  • Collaborate with healthcare professionals to ensure seamless care coordination.
  • Provide health education and counseling to patients and their families.
  • Maintain accurate and timely records of patient encounters.

Requirements:

  • Master's degree in Nursing or related field.
  • Current RN and NP licensure in state of practice, including prescription authority.
  • Board certification by AANP, ANCC, or AACN in a Matrix-approved specialty.
  • Minimum 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health records.
  • Excellent verbal and written communication skills.
