Schedule: Full-Time, 40 hours per week

Shift : Days, primarily Mon-Fri, rotating weekends.

Location: This position will perform patient home care visits in the Hillsborough County area.

The role youll contribute:

The Home Health Speech Therapist Case Manager is a professional therapist who coordinates and directs the home care patients care based on individual patient needs. Case Manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. The therapist is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience is consistently applied to new patient populations. The Case Manager cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician. S/he educates patients, families, caregivers and community providers to safely perform care. S/he provides follow up by evaluating effectiveness of the home care plan, and monitoring patient/familys response to the plan to achieve patient/family goals and top decile outcomes. The Case Manager also identifies performance improvement and home health standard of care initiatives and assists to design or implement programs to address needed changes.

  • Performs an evaluation and utilizes the findings to determine diagnosis (for SLP services), prognosis and plan of care in consultation with the physician.Determines specific interventions, duration and frequency needed to achieve anticipated goals and outcomes.
  • Observes and records activities and findings in the clinical record and reports to the physician and or primary Case Manager the patients reaction to treatment and any changes in patients condition, or when there are deviations from the plan of care.
  • Instructs the patient and caregiver regarding the patients speech language improvement program including adaptive equipment and the home program.
  • Instructs other health team personnel including, when appropriate, home health aides and caregivers in certain phases of speech therapy with which they may work with the patient.
  • Selects, applies or modifies skilled intervention to the patient using various skilled interventions consistent with the state practice act to achieve the best outcome possible for the patient.Assesses patient response to interventions and performs reassessments as required.
  • Documents in medical record, accurately reflecting treatment provided.Updates the care plan as needed including the aide care plan; writes modified orders for changes in the plan of care, performs and documents the aide supervision by the due date.
  • Completes all documentation within the time frame required by the agency, including reassessments, discharge summaries and OASIS documents.
  • Participates in clinical record review, agency staff meetings, therapy meetings and other activities as requested.

The expertise and experiences youll need to succeed:

Minimum qualifications :

  • Minimum of one-year relevant clinical Speech therapy experience
  • Masters degree in Speech Therapy
  • Current Speech Therapy License in State of Practice
  • Valid Drivers License and current car insurance
  • BLS certified

Preferred qualifications:

  • Recent, relevant experience in a Medicare-certified home health agency as a case-manager
  • Home Health Case-Manager Certification
  • COS-C Certification

Supervisory responsibilities:

  • Field supervision: Effectively delegates and supervises delegated care, e.g., care provided by the home health aide and speech therapy assistants.
