Assessment: Conduct regular assessments of the client's health status and report any changes to the healthcare team. Medication Management: Administer medications as prescribed and monitor for side effects or adverse reactions. Wound Care: Perform wound care and dressings as needed. Dressing Transfer Mobility Vital Signs Monitoring: Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature. Documentation: Maintain accurate and detailed records of patient care, observations, and treatments. Education: Educate the client and their family on healthcare practices, self-care techniques, and disease management. Communication: Communicate effectively with the healthcare team, client, and family members to ensure coordinated and continuous care. Compliance: Adhere to all federal, state, and local regulations and guidelines, as well as the policies and procedures of ATN Healthcare. Supervise CNAs, Home Health Aids, and care givers. Qualifications: Licensure: Current and valid LPN license in the state of Colorado. Education: Graduation from an accredited LPN program. Experience: Minimum of one year of experience in home health care preferred. Skills: Strong clinical and assessment skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Proficient in electronic health records (EHR) and documentation. Certifications: Current CPR certification. Transportation: Reliable transportation and valid driver's license. Physical Requirements: Ability to lift and move patients as required. Prolonged periods of standing, walking, and moving. Ability to work in various home environments. Work Schedule: Flexible scheduling with the potential for weekends, holidays, and on-call duties as needed.
