**About our Foster in Texas (FIT) Program**

Upbring Foster In Texas is one of the largest providers of foster care and adoption services in Texas with 13 offices across the state. In 2021 we served 1,176 children, verified 140 new families, and connected 113 children with loving adoptive families. The Upbring heroes on our foster care team connect children with families who provide compassion, safety and support, and when needed, loving forever families.

**What You'll Do as a Registered Nurse Primary Medical Needs Treatment Director**

In collaboration with the interdisciplinary team, service professionals, and community partners, the Primary Medical Needs (PMN) Treatment Director provides, supports, plans, develops, implements, and monitors individual medical needs plans for foster children assigned to a caseload in accordance with Agency policy, licensing standards, and local, state, and federal regulations. An important role of the PMN Director is to oversee, guide, and support the Licensed Vocational Nurses (LVNs) employed by the program.

**Registered Nurse Primary Medical Needs Treatment Director Work Standards**
- Provide direct and indirect care within the scope of RN duties and responsibilities as outlined in the established policies and procedures, standards of care, and other regulatory and licensing standards
- Provide oversight, guidance, and support to LVN staff to ensures the child and homes in need of primary needs are provided in compliance with treatment plans and licensing and regulatory standards
- Conduct the initial assessment by assessing the child’s current medical/health status and past and present treatment plans and services to determine primary medical needs
- Conduct follow-up assessments by assessing the child’s current medical/health status and past and present treatment plans and services to determine primary medical needs
- Utilize assessment tools and skills to develop and implement a treatment plan that meets specific objectives and goals and needs of the child; provide intervention plans if needed
- Monitor and report changes in child’s condition, reaction to treatment plan, and progress, document changes according to agency policies and licensing and regulatory standards
- Visit and monitor the health and well-being of hospitalized foster child and adhere to the prescribed treatment plan upon discharge Participate in nursing consultations, as requested, for children placed in the program
- Regularly review, monitor, and evaluate the program to identify and minimize risks
- Other duties and projects as assigned

**Communication**
- Review and update medical records as required by regulatory and licensing standards
- Obtain medical and immunization records from available resources and/or systems
- Establish and maintain frequent communication with the child placement unit at DFPS or SSCC
- Conduct and share results of clinical audits with the interdisciplinary and treatment teams

- Coordinate services with interdisciplinary and treatment teams, including attending physician, specialists, medical and non-medical providers to meet the primary medical needs of the child
- Consult with the interdisciplinary and treatment teams to determine medical needs not deemed as primary medical needs
- Work in collaboration with interdisciplinary and treatment teams to evaluate the medical and developmental needs of the foster child
- Work with child placement management staff to review and coordinate admissions of children identified as primary medical needs
- Work with Clinical Director and other agency staff to prepare and submit clinical documentation for third party review to determine appropriate level of care
- Provide on-call medical needs services
