**Job Summary**:
We are seeking a compassionate and dedicated Home Health Aide or Caregiver to join our team. You will provide essential care and support to patients at home. This is a rewarding opportunity to make a positive impact on the lives of individuals in need.
**Responsibilities**:
- Assist patients with activities of daily living, such as bathing, dressing, toileting, transferring and grooming
- Assist with housekeeping tasks such as meal prep and laundry
- Transport and accompany clients on errands or medical appointments
- Provide emotional support and companionship to patients
- Maintain accurate and up-to-date medical documentation
- Follow infection control protocols to maintain a safe environment
- Adhere to policies and procedures set forth by the office administration and nurse
**Skills**
- Experience in home care, memory care, acute care, senior care, or dementia care is preferred
- Ability to provide compassionate care to patients
- Strong communication skills to effectively interact with patients, families, and healthcare professionals
- Ensure patient confidentiality and discretion in all settings
**Requirements**:
- Driver’s License
- Current CPR
- Current negative PPD / TB test results (results must be read within the last year)
- An additional form of identification
- Driving Record
- Declaration page of auto insurance
