**DESCRIPTION**
Adheres Agency’s admission policies and procedures; coordinates tracking of referral information for the ongoing six month reporting to DCFS; receives referrals for foster family home placement of children, obtaining relevant clinical, collateral and financial information requisite to determining service needs for the child and his/her family; makes preliminary recommendations to the interdisciplinary team concerning appropriate foster home placement; facilitates admission staffings and subsequent assignment of the child to a case manager.

**QUALIFICATIONS**
Bachelor’s degree required in social work, psychology, education or other human service field. Minimum of one year successful work experience with persons with disabilities or social service field/preferred. Bi-lingual (English/Spanish) Preferred. Valid Driver’s license, and Vehicle with Proof of Insurance.

Pay: $38,000.00 - $45,000.00 per year
