This position is responsible for educating, screening, and referring parents/children about Adverse Childhood Experiences (ACEs). In addition, this position works closely with the integrated health team, medical provider, and medical assistants for care coordination. The ACEs Project Assistant will have the opportunity to meet with community partners for follow-up updates and needs.

**_ Essential Functions_**
- Monitor medical provider’s schedule to flag patient’s requiring ACE screening.
- Coordinate with Medical Assistants on when to step into patient’s room to discuss and screen for ACEs
- Educate and review ACEs questionnaire and determine if a Behavioral Health (BH) consult is required
- Coordinate with Integrated Health Services Assistant to schedule BH consult whether that be in person, tele-visit, or Healow visit
- Initiate and follow up on any referrals sent to outside facilities
- Maintaining and distributing ACEs educational information
- Regular reporting to Behavioral Health and Pediatrics Provider to create a streamlined workflow for ACEs screening and intervention
- Record keeping and intermittent reports on ACEs screening and parameters.
- Consistently ensures knowledge of and complies with Mountain Park Health Center policies and procedures
- Observes and respects the confidentiality information regarding patients, visitors, vendors, and co-workers
- Maintain consistent, timely communication regarding all facets of departmental activities with his/her co-workers
- Must be able to embrace differences among people and is able to interact with internal staff as well as external contacts in a culturally competent and respectful manner.

**_ Position Qualifications_**
- High School Diploma/GED
- Must be bilingual and able to interpret in Spanish
- Preferred Qualifications: Electronic Health Record experience
