We are seeking a **Clinical Team Manager** to serve as a leader and resource for the Home Health Department on a day-to-day basis. This role is responsible for coordinating high quality patient care using an interdisciplinary team approach.

**Essential Duties & Responsibilities include **(but are not limited to):

- Provide guidance to the team members in regular transdisciplinary team meetings; the development, review and revision of the patient/family plan of care; facilitation of problem solving sessions; and delivery of team support.
- Assess daily staffing needs and implements re-distribution of resources based on census to maximize patient care.
- Periodically audit selected clinical records for documentation concerning HHA supervision, visits/matching orders, and other areas related to regulatory compliance and enter findings in audit database. Hold staff accountable to standards.
- Coordinate transdisciplinary team meetings and supervise the planning, coordination, and delivery of the transdisciplinary team interventions.

**Education/Experience**:

- Bachelor's Degree in Nursing, Social Work, or Physical Therapy.
- Current active CA state license in their clinical field.
- Minimum of two years of nursing, social work practice, or physical therapy experience ideally in a home health, hospice or Community health setting within the past five years.
- Five years supervisory experience or relevant progressive management experience preferred

**Certificates, Licenses, Registrations**:

- Current California Driver's License and automobile insurance that meets State of California requirements, if driving a car.

**Pay Range**:
$130K - $156K annually
