**Responsibilities**:

- Complete psychosocial assessment (appropriate to patient type and reason for referral) to assist in development of a care plan.
- Provide brief, targeted intervention for identified patients.
- Provide education/consultation to patient/family, treatment team and community.
- Maintain accurate and current notes in both the Mental Health records and patient records of all patients seen
- Integrate practice and institutional expectations.
- Provide timely continuing care planning.
- Actively collaborate with case management and health care team in identification of outcome indicators, in data collection and analysis and in subsequent system improvement.
- Provide individual and/or family therapy for identified patients
- Formulate and conduct educational, therapeutic, and/or support groups
- Contribute to the training experience of mental health technicians.
- Liaison to community resources, programs.
- Crisis response and follow-up care in the event of unit casualties or other serious incidents.
- Participate in and utilize supervision to assess level of competence in clinical social work, identify areas for professional growth and establish mutually agreed upon goals.
- Treatment planning using evidence based psychotherapeutic interventions.
- Consult with medical personnel, legal authorities and military commands as appropriate.

Qualifications:

- Masters of Social Work (MSW) degree from an accredited graduate school of social work
- A minimum of 2 years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years
- Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care;
- Facilitation: Care coordination and communication among all involved parties;
- Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care coordination is received.

License - Certifications:

- Current/maintained unrestricted license in clinical social work in any one of the 50 states. License cannot be under investigation nor have any adverse action pending from a State Board or national licensing/certification agency.
- Current Basic Cardiac Life Support (BCLS) certification.
