We are seeking a dedicated Hospice Social Worker to join our healthcare team. The ideal candidate will provide essential social work services to patients, families, and caregivers in a home hospice setting.

**Duties:**

- Conduct psychosocial assessments of patients and families to identify emotional, social, and financial needs

- Develop and implement individualized care plans in collaboration with the healthcare team

- Provide counseling and support to patients facing medical challenges and their families

- Coordinate end of life and discharge planning

- Advocate for patients' rights and access to community resources

- Maintain accurate and timely documentation of patient interactions

**Qualifications:**

- Bachelor's or Master's degree in Social Work from an accredited institution

- State licensure as a Social Worker (LCSW) preferred

- Knowledge of medical terminology, and HIPAA regulations

- Experience working with geriatric populations and patients with complex medical conditions

- Familiarity with crisis management techniques and addiction counseling

- Proficiency in cognitive behavioral therapy and applied behavior analysis

- Strong communication skills and ability to collaborate effectively with interdisciplinary teams

- Compassionate attitude towards patient care
