Summary:
In the event of a staff calling in last minute, it will your responsibility to cover the shift or find a replacement.
Provide Direction & Guidance to staff and follow up on duties assigned.
Maintain Communication with Supervisor, APD, Families and Supports.
Prepares Home for Annual Inspections (QLARANT, APD & CMS).
Serve as a key member of management team and work in close concert with the Operations Manager.
**Job Description**:
- Assist with New Admissions - Transfers &/or Discharges
- Serve as Court Liaison for consumers requiring legal follow up visits
- Coordinate groceries & supplies, maintain adequate supplies, food, linen & safety equipment
- Maintain consumer’s hygiene, rooms & house in a TOUR READY status = neat & clean
- Coordinate & attend Annual Support & Behavior Plan Meetings with WSC’s/BA’s and Oversight
- Schedules, attends and transports individuals to medical, dental, psychiatric and counseling appointments
- Complete documents related to incidents reports, behavior issues family issues, illnesses
- Participate actively in the individual’s services by providing ideas/suggestions
- Provide training to new and existing staff for group home operations on a regular basis
- Keep Residential Supervisor aware of situations - especially Health & Safety issues
- Keeps all consumer binders and forms current by coordinating with Supervisor
- Conducts fire drills monthly and updates individual evacuation plans
- Assist with the development and follow monthly activity calendars for the residents
- Verify Meds & MAR’s are correct when delivered by pharmacy & update changes
- Report all maintenance issues in a timely manner & document in the Maintenance log
- Ensure the vehicles are locked, kept clean and report hazards & maintenance issues
- Cover staff call offs & fill in positions and notify the operations manager
- Maintain Effective Communications with Management in a timely manner
- Attend or Delegate community activities/know whereabouts of all consumers at all times.
- Report & respond to emergencies and know homes electrical & plumbing shut off systems.
- Maintain 24 Annual Training Requirements
- Maintain order & safety of the home including all repairs needed.
- Serve as Liaison to Consumers’ Guardian, Families, WSC’s and Supports
- Attend Individual School IEP Meetings
- Check & Follow Up On: End - Of - Shift, Phone Reports and weekend events
- Conduct monthly meetings with staff for accurate management of services and staff are aware of their roles for time sensitive written reports and where all said items are required to be securely stored: Daily Service Logs, Parent Concern forms and Incident & Medication Error Reports, Monthly Staff & Consumer Meeting, Daily Attendance Log, Menu & Substitution Menus / Special Dietary Needs, Companion notes, Transportation Logs and Any other documentation required by the Agency, APD and QLARANT
- Work closely with Operations Manager regarding Scheduling, Resources and/or Staff issues
Job Qualifications:
2-3 years residential support services/caregiver preferred
Must be able to pass the necessary ADP Trainings
Must possess a valid drivers' license
Must be able to pass a level 2 background check
Abilities:
