**JOB TITLE: Scheduling Coordinator**

**JOB SUMMARY**

The coordinator is responsible for matching the best caregiver with the appropriate client and managing the ongoing patient needs keeping continuity as the priority aspect of the process. Reporting employee issues including but not limited to no call no show, unreliability, failure to follow the schedule, changes days/times of visits without notification are critical to the success of the department’s standards.

**QUALIFICATIONS**
- A minimum of Three (3) years recent experience as a scheduling coordinator in a home care agency or other healthcare field.
- Excellent customer service and communication skills to provide scheduling coordination for homemakers and companions
- Ability to multi-task and a propensity for working with and helping people.
- Must have a criminal background check.

**RESPONSIBILITIES/ESSENTIAL FUNCTIONS**
- Develops new processes to improve workflow and efficiency and reinforces compliance with existing processes.
- Manage weekend schedule including but not limited to phone inquiries, callouts and patient /caregiver requests.
- Receives all complaints/problems related to off-hour issues or on-call.
- Schedules caregivers to assist clients in a timely manner.
- Notifies the appropriate and responsible staff of caregivers’ emergencies, sickness, and other imminent occurrences that may affect the client.
- Ensure compliance for caregivers written time off request 2 weeks or more in advance for planned time off.
- Actively participate in caregiver orientation program.
- Lead point of contact for clients and caregivers requesting home care.
- work with care mangers to ensure client caregiving needs are met.
- Participate actively with new client intake process.
- Communicate schedule changes to clients and family members.
- Task client care manager via Clearcare.
- Process other duties as assigned.
