**Care Manager/Case Manager Job Responsibilities**:

- Manages an assigned caseload of program members in collaboration with an RN Care Manager
- Completes comprehensive member assessments, member centered care plans, & functional screens
- Provides ongoing care coordination and quality assurance monitoring of services
- Maintains required documentation in adherence to established standards.

**Care Manager/Case Manager Job Requirements**:

- **Education**: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
- **Experience**: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
- **Certification**: Eligibility for long-term care functional screener certification is required
- **Travel**: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

**Care Manager Job Schedule**:

- Monday to Friday, 8:00 AM-4:30 PM
