**Standard Employment Requirements**

- Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
- Must pass substance abuse testing upon employment and submit to random testing during employment.
- Must provide an updated immunization (IZ) record at the time of employment.
- Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
- Must obtain a First Aid/CPR Card within (90) calendar days of employment and maintain a valid CPR Card during employment.

The Community Health Worker (CHW) with the Arizona State Maternal, Infant, and Early Childhood Home Visiting Program (MIECHV) program provides assistance and support in obtaining health related services for clients of NATIVE HEALTH and the Prenatal Clinic as well as Women, Infants and Children (WIG). The CHW is a liaison between the Arizona Department of Health Services, the health care provider, and the Phoenix Indian Community.

**KEY ACCOUNTABILITIES**:

- Deliver services primarily through prescheduled home or office visits and scheduled health related classes.
- Identify and enroll pregnant and postpartum women, caregivers, and their children in the program in accordance with the Arizona MIECHV policy and procedure manual.
- Participates in training identified to be consistent with program objectives to provide education to benefit enrolled participants.
- Documents and keeps records of all visits and case management services provided to families.
- Supports the organization as needed.

**Qualifications** QUALIFICATIONS/EXPERIENCE/REQUIREMENTS**:

- CHW must live in the identified neighborhood/community and reflect the characteristics of the community.
- High School graduate or GED equivalency.
- Has at least 2-3 years teaching experience in women's & children's health.
- A sincere desire to improve the health status of the urban American Indian population and NATIVE HEALTH's community.
