**The Social Worker/Clinical Case Manager shall assist the Wellness Director and Nursing Staff to ensure participants’ medical, social, and psychological needs are met by providing thorough and effective needs assessments and coordination of outside community resources.**

**Reporting Relationships**:
The Social Worker/Clinical Case Manager reports directly to the Wellness Director

**Responsibilities**:

- In partnership with the Wellness Director and the Nursing Staff, the Social Worker/Clinical Case Manager reviews clinical information obtained by the prospective resident’s medical providers in advance of move-in to determine appropriateness
- Interviews new and existing residents to assess psychosocial needs and provides emotional support and guidance as needed
- Contributes to service planning during weekly Multidisciplinary Meetings and aids in family meetings, hospice meetings, discharge planning meetings and new partnership meetings with visiting nurse agencies in the community
- Assists the Wellness Department with follow up on medically absent residents in whichever setting they find themselves rehabilitating within—hospital, short-term rehabilitation, in-patient psychiatric care
- Coordinates community services consistent with participant’s needs (i.e. adult day health, psychiatric services) in person or virtually
- Documents visits as well as any concerns or incidents in Move-N Software and works with the Wellness Department to determine resolution
- Speaks with residents’ families or responsible parties, when permission is given, to update them on status, suggestions for course of action, and supplies needed
- Aids with case management of all residents, including those participating in Group Adult Foster Care (GAFC), The Department of Developmental Services (DDS) ABI/MFP Waivers, Fallon Navicare and Senior Whole Health (SWH), by completing monthly visits, appropriate documentation, and assisting them in maintaining communication with case managers, program coordinators, and primary care physicians/specialists
- Assists with incident reporting to the DDS for ABI/MFP Waivers as well as Bristol Elder Services (BES) Protective Division
- Maintains confidentiality of pertinent resident care information to assure residents’ rights are protected
- Networks with local hospitals, continuing care coordinators, nursing homes, home health agencies and rehabilitation facilities, as needed, in conjunction with the Wellness Department
- Fields insurance-related questions and provides resources and support as needed
- Completes a monthly summary of interactions with ABI/MFP waiver clients
- Coordinates ISP meetings with DDS staff
- Works with the Reception staff in scheduling appointments and coordinating transportation for residents using transportation options through MassHealth, Navicare, Senior Whole Health, or the Whaler’s Cove Van
- Assists the Wellness Department with preparing end-of-day reporting and medical record maintenance/compliance.
- Such other duties as may be requested by the Wellness Director or Executive Director

**Qualifications and Competencies**:

- Bachelor’s degree in social work or related field
- 1-2 years of related experience, with a minimum of one year with the elderly
- Knowledge of social work functions in working with the elderly
- Excellent verbal and written communication skills
- Computer skills
- Ability to work independently and is sensitive to the needs of the elderly
- Flexible schedule
- Familiarity with the surrounding community and its resources a plus
