**Job Summary**

The Client Care Manager will ensure accuracy and timely compliance of all procedures, regulations, and policies pertaining to the client intakes, intake documents, case management policies, etc. Under the general supervision of the Executive Director, the Client Care Manager is responsible for performing case management, referral and assessment/evaluation of consumers inFort Bend and Waller County to determine in-home service (meals on wheels) eligibility, including performing supervisory duties over the in-home service staff providing leadership, direction, and long range and short term planning, directing policy development and performance criteria for the assigned area(s). This position also works with the Executive Director on any other projects as necessary. These duties are always performed with the agency’s mission in mind.

**Qualifications and Skills**
- Bachelor’s degree from a four-year college or university or equivalent experience
- 3 years related job experience, preferably working with the elderly population
- Strong computer skills; specifically, Microsoft Outlook, Microsoft Word, Microsoft Excel, Windows, and Google Chrome
- Excellent telephone, communication, and writing skills
- Ability to interact and communicate effectively with other employees, volunteers, clients, families/caregivers, outside organizations, and the public
- Highly organized, detail oriented, excellent problem-solving skills and ability to make good judgment decisions
- Valid Texas Driver’s License and excellent driving record
- CPR and First Aid Certified (Certification can be obtained after employment)
- Ability to adapt to frequent change and work in a fast-paced environment

**Ability**
- To oversee/perform case management for in-home services (meals on wheels)
- To obtain facts and analyze data
- To prepare clear, concise, and accurate records and reports
- To communicate effectively - both written and oral communication
- To establish and maintain client rapport on an individual basis
- To analyze and interpret public social service programs, policies, and procedures for applicants and recipients
- To maintain client confidentiality and case record confidentiality
- To effectively represent the In-home Service program in contacts with other employees, volunteers, clients, families/caregivers, community organizations, government agencies, and the public
- To establish and maintain cooperative working relationships

**Essential Duties and Responsibilities**
- **Client Care Manager**_
- Participate in the development of long-term and short-term plans and goals for the assigned area, establishes and/or monitors current methods and policies, and keeps management level personnel abreast of trends and issues in the field and in the department
- Builds and maintains effective relationships with public and private community-based service providers and professional and community groups in order to exchange information, resolve concerns/complaints, and develop and maximize available resources for information and referral
- Represent the department on a variety of internal committees and acts as a project lead on internal and external committees/groups
- Develops or modifies work plans, methods, and procedures and determines work priorities
- Resolves problems encountered during daily operations and determines standards for problem resolution
- Provides work instruction and assists employees with difficult and/or unusual assignments pertaining to case management
- Compile, analyze and provide weekly and monthly reporting to ED and CFO
- Performs other related duties as assigned
- **In-Home Services Case Management**_
- Assessment and reassessment of potential clients in their home to determine eligibility for meals on wheels program
- Provide initial screening of potential clients over the phone
- Provide information and referral services for seniors
- Reassess clients a minimum of at least every twelve months in the client’s home
- Maintain all client files in accordance with the policies and procedures of FBS and funding agencies
- Comply with the appropriate local, state, and federal laws, and policies, regulations, and procedures set forth by the funding agencies, foundations and FBS
- Maintain the allowable number of Meals on Wheels clients
- Attend all staff training and in-service workshops as required
