The Peer Mentor will serve as a guide in order to assist clients with navigating the community.

**DUTIES AND RESPONSIBILITIES**:

- Assist Case Managers in delivery of services
- Establish relationships with families of client
- Establish relationship with potential clients
- Assist in planning of activities for clients which includes but not limited to:

- Workshops
- Community Events
- Field Trips
- Creates and implements activities/workshops for clients and families
- Provides a long term plan of action for each clients which includes direct services and referral services, including, neighborhood-based case management, relocation and transition, educational support, referrals to housing, mental health, substance abuse treatment, and tattoo removal etc.
- Advocates for the client when needed
- Enroll clients in to job/school and other services needed to remain compliant with parole requirements
- Communicate with team on the daily basis regarding client progress
- Works with parents/guardians/siblings and provides services to the family, if needed
- Coordinates all activities for the client and families with the various stakeholders in the person’s life including school officials, employers, probation officers, law enforcement officials, etc.
- Monitors agencies to which clients are referred to assure quality service
- Works collaboratively with other case managers
- Coordinates and collates monthly program reports
- Performs related duties as required.*

**REQUIREMENTS**
- Fully vaccinated for COVID-19 as outlined in the contract requirements
- Comply with Hospital Background/Clearance as outlined in the contract requirements

**QUALIFICATIONS AND EXPERIENCE**:
Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:

- 1 to 3 years of experience in mentoring and providing resources and services to high risk youth
- Commitment to the mission and values of the non-profit sector, with a passion for social impact and community service
- Must have reliable transportation, a valid CA Driver’s License, and proof of auto insurance
- Must be able to successfully pass Live Scan (background check)
- Required TB Test clearance
