**Summary**: This community-based position involves providing direct, face-to-face services to individuals experiencing homelessness in various locations across Sacramento. The Outreach and Engagement Worker is responsible for delivering mental health, substance abuse, and supportive services, aligning with the Psychosocial Rehabilitation & Strengths Model, tailored to an adult population facing homelessness and mental health challenges. The role encompasses recovery and strengths-based community outreach, case management, housing navigation, social rehabilitation, skills building, benefits and resource support, group facilitation, and crisis intervention.

**Essential Duties and Responsibilities**:

- **Provide Outreach and Engagement Services**:

- Engage individuals experiencing homelessness within the designated outreach service area.
- Educate, inform, and connect individuals to Mental Health services and community resources addressing immediate needs like food banks, clothing closets, 211, and shelters.
- Respond to community outreach requests directed to CORE sites regarding individuals experiencing homelessness.
- **Client Care Coordination**:

- Coordinate care and collaborate with all parties involved with the client and natural supports.
- Work with faith-based, fraternal, cultural, etc.based Community-Based Organizations (CBOs) to benefit individuals who may be homeless within outreach zones.
- Collaborate and potentially co-deploy with local government agencies for seamless warm hand-offs.
- **Short-Term Assistance and Eligibility Screening**:

- Provide short-term assistance to support housing readiness.
- Screen individuals face-to-face for eligibility and enrollment into the program.
- **Advocacy and Liaison**:

- Advocate on behalf of clients to ensure fair and just treatment in securing benefits and housing.
- Collaborate with law enforcement agencies to address potential community needs.
- **Documentation and Reporting**:

- Track all outreach activities and submit reports to the Program Director monthly.
- Prioritize and adjust to client and clinic needs to create quality services.
- **Educational Outreach and Support**:

- Distribute educational materials, donated items, and outreach kits/supplies to homeless individuals.
- Contact and inform individuals in various community settings about community programs in Sacramento County, including CORE services.
- Provide community stakeholders with educational information on behavioral health and social service programs.
- **Transportation and Support at Community Wellness Center**:

- Provide transportation for clients to attend mental health/medical/benefit appointments.
- Support clients/guests in the Community Wellness Center, conducting HMIS assessments and service data entry for those experiencing homelessness, chronically homeless, or at risk of homelessness.
- **Participation and Training**:

- Participate in supervision, staff meetings, consultations, travel, crisis/emergency coverage, weekend coverage, and agency-wide meetings and training, as required by the supervisor.
- **Charting and Documentation**:

- Maintain all charting records and documentation in accordance with El Hogar and Sacramento County regulatory policies and procedures.
- Produce the required number of regulatory units of service per month (70% of staff’s time must be providing face-to-face or other eligible community-based support with/for the client).

**Supervisory Responsibilities**: This position does not involve any supervisory responsibilities.

**Education and/or Experience**: Minimum qualifications can be met in the following ways:

- High School Diploma or GED (required) AND
- Four (4) years of experience in a Mental Health or Drug and Alcohol Rehabilitation related field providing services to individuals experiencing mental health challenges and/or homelessness. OR
- Bachelor’s degree in a related field.

**Certificates, Licenses, Registrations**: Current, valid California Driver's License and driving record acceptable to El Hogar and the Agency’s auto insurance carrier. Current proof of personal auto insurance.

El Hogar offers a comprehensive range of employee benefits, with considerations for both full-time and part-time employees. Benefits are pro-rated for those working less than 40 hours but more than 20 hours per week. The benefits include:
