The Dane County Sheriff's Office has two (2) full-time (1.0 FTE) vacancies and one (1) part-time (0.5 FTE) vacancy for the Re-Entry Coordinator (Project) position. One full-time position is 10am - 6pm and one full-time position is 3pm - 11pm. Both full-time positions have the ability to flex hours. The part-time position will be weekends and some weekdays.

These positions are funded for one year through a grant with the potential for position(s) to receive continued funding through the county budget process.

The eligible list created from this posting will be used for future Re-Entry Coordinator vacancies for the next six (6) months.

**DEFINITION**
Under the supervision of the Security Services Lieutenant, provides information and assistance connecting jail residents to programs and community services, to include public and privately funded options. Provides assessments and short-term follow up supportive services to residents who are being released from the Dane County Jail. Develops case plans which build on the strengths of the individuals, families and community resources. Works collaboratively with internal and external services in order to advocate for residents to secure services that appropriately fit their needs. This position will require evening hours and some weekends. This position is funded for one year through a grant with the potential for position(s) to receive continued funding through the county budget process.

**EXAMPLES OF DUTIES**
Provide intake, screening and assessment for residents incarcerated at the Dane County Jail to determine strengths and immediate needs, primarily focused on those who are incarcerated for short periods of time; develop client centered case plans to address immediate and longer term needs; supplying tangible resources and referrals to agencies that can fulfill the needs. Organize supportive services for the individuals to utilize once released from the jail. Work closely with community agencies, treatment personnel, and other appropriate services providers. Assist in the coordination and facilitation of services for the client’s reentry plan. Maintain a caseload of clients who will be releasing from the jail. Complete referrals for a number of different agencies statewide. Conduct assessment and placement interviews for residents seeking treatment or long term transitional care. Utilizing databases, written and computer based informational resources to identify, evaluate and refer to programs. Maintain excellent and diligent paperless documentation of services provided, and maintain client files using several database systems.

**Education, Training and Experience**:
Individuals must possess a Bachelor’s Degree in social work, human services, criminal justice, psychology, or a closely related field. Individuals with a Bachelor’s Degree must also possess at least two (2) years of professional experience working with any of the following: Criminal offenders’ rehabilitation work; Ex-offender client population in the criminal justice system; Restorative justice practices (the restorative justice practice can be in the following areas: criminal justice system, community-based program.); or facilitating programs and services for at risk populations.

**Special Requirements**

Valid Wisconsin driver’s license (or eligibility for), access to personal transportation is required. Lifting 20 pounds maximum with frequent lifting and/or carrying of objects. Maintain driver’s license and access to reliable transportation while employed in position.
