Job Duties and Responsibilities for Housing/ Mental Health Case

Manager and Coach

Emergency Housing Assistance:

- Work with displaced families and youth to secure emergency

housing.
- Conduct thorough assessments of housing needs and

vulnerabilities.

Legal Assistance:

- Assist clients in securing legal representation in

contesting evictions.
- Provide guidance and support in understanding tenant rights

and responsibilities.

State Aid Programs:
programs related to housing and financial assistance.
- Ensure clients are aware of and have access to available

resources.

Client Progress Monitoring:

- Follow up with clients regularly to ensure their continued

success and stability in housing.
- Monitor clients' progress towards housing goals and adjust

strategies as needed.

Treatment Planning:

- Accomplish clients' care by assessing treatment needs.
- Develop, monitor, and evaluate treatment plans and

progress.
- Facilitate interdisciplinary approaches when necessary.
- Monitor staff performance in delivering services

effectively.

Housing Search Support:

- Provide housing search support to families and youth by

assisting them in locating appropriate housing rentals.
- Stay updated on available housing resources and options in

the community.

Community Engagement:

- Attend appropriate community meetings and collaborate with

other agencies and organizations to meet housing program

outcomes.
- Foster relationships with landlords and housing providers

to enhance housing options for clients.

Documentation and Reporting:

- Complete and maintain client files as indicated by the file

content checklist. Keep accurate, complete, and timely data and

reports for program evaluation and reporting to the Program

Manager.

Client Advocacy:

- Keep the Program Manager informed of issues and concerns

related to clients.
- Advocate for clients' needs and interests within the

program and the community.

Program Evaluation:

- Participate in program evaluation efforts to assess the

effectiveness of housing services.
- Provide input and feedback to improve program outcomes.

Data Management:

- Ensure the proper recording of data for program outcomes.
- Maintain up-to-date and accurate client files.

Family-Specific Responsibilities (if applicable):

- Provide housing case management and direct client services

to families, including housing search support.
- Conduct 1:1 case management activities to assist youth in

meeting service goals.
- Coordinate with the Homeless Management Information System

(HMIS) for data collection.
- Facilitate educational, vocational, employment, and career

development activities for youth.
- Offer independent living and life skills training to

youth.
- Connect youth with appropriate mental health and/or

chemical dependency support.
- Encourage youth and families to participate in youth

development activities.
- Provide follow-up and aftercare services to youth as

needed.
- Assist with housing placements and transitions for

families and programs.

Other Duties:

- Perform any other duties as assigned by the supervisor or

program leadership to support the mission and goals of the

organization.

These duties and responsibilities encompass the critical roles

and functions of a Housing Case Manager and Coach, focusing on

assisting clients in securing safe and stable housing while

addressing their unique needs and challenges.

Documentation Compliance:

- Ensure that all client documentation, including case notes,

assessments, and progress reports, is completed within 48-72

hours after the service is provided.
- Accurately record all client interactions and interventions in

the Electronic Health Record (EHR) database, such as Sharenotes,

to maintain comprehensive and up-to-date records.

Client Engagement:

- Maintain a regular schedule of face-to-face meetings with

families, meeting with each family at least 1 to 2 times weekly.
- Establish a strong rapport with clients through regular

in-person interactions to build trust and provide ongoing

support.

These additional duties emphasize the importance of timely and

accurate documentation in the EHR database and highlight the

frequency of face-to-face interactions with clients to ensure

their needs are met effectively and consistently.
