BASIC FUNCTION:

- This position participates in community events to increase community awareness of clinic programs, expand HIV and HCV testing and treatment, educate the public on HIV/HCV/PrEP and work to reduce related stigma. Serve as the liaison for clinic patients living with HIV, HCV or taking PrEP, as an organization, and community services. Conduct HIV/AIDS tests and provide pre and post test counseling and provide accurate information to clients about living with HIV, preventing further transmission, the benefits and challenges of HIV care, Pre-exposure Prophylaxis (PrEP) and available services.- SUPERVISORY ACCOUNTABILITY:
- Responsible for providing limited supervision for one or more functions within a department. Formally plans, assigns, directs, and coordinates the work of these functions. Typically responsible for performing some non-supervisory duties in addition to supervisory responsibilities. May perform staff evaluations and make recommendations regarding pay and/or performance- NATURE AND SCOPE:
- Internal contacts include administrators, faculty, and staff both- within the department and University-wide. External contacts include government agencies, sponsor representatives, vendors, visitors, officials and the general public.- PRINCIPAL ACCOUNTABILITIES:
- Activities for this position will include, but not be limited to screening and enrollment for eligible community programs; patient mentorship; patient education; patient follow-up and reinforcement of treatment plan tasks.
- Explain diagnostic lab test results Coordination of appointments and transportation, and patient engagement.
- Foster cohesive relationships with staff and clinical teams, with community partners.
- Work with an interdisciplinary team of physicians, care managers, social workers, and other community health workers to improve health outcomes.
- Active in community engagement activities to include working with community organizations and assisting with community-based activities.
- Must be able to operate a vehicle for travel expectations within the community.
- Must have reliable daily transportation for work related travel (to and from community testing sites, patient home visits, shelters, meeting sites, clinics, Department of Health STI clinic, etc.).
- Visits individuals and families in their homes, in shelters or other similar places to provide basic community outreach services and to assist with social interventions necessary in coping with a new diagnosis.
- May interact with individuals and families at the bedside in the hospital to enroll in community outreach services. Once enrolled, primary communication with patients may be telephonic, face to face or via written material. Community Health Workers will collaborate closely with medical providers, primary care teams and PrEP Coordinator to improve patient care.
- Provide support when needed with enrollment/re-enrollment into Medicaid, Medicare, SSI, or SSDI in collaboration with EIS Coordinator and PrEP Coordinator.
- Provide informal counseling, social support, and appropriate referrals, in consultation with clinical staff when appropriate, to ensure patients have access to services needed.
- Provide care support, follow-up, and education in clinical and community settings by identifying and addressing issues that create barriers to care for patients, they will facilitate patients’ ability to receive services.
- Provide follow up with the patient in person and/or via telephone to ensure the patient was able to fully access services and have their issues addressed.
- Collect relevant data on activities, patients, and report that data in the means required by funding sponsor and clinic electronic health record and data management protocols.
- Must possess an understanding of cultural competency in providing effective prevention services.
