Hospice of the East Bay (HEB) is a community-based hospice and palliative care program providing services in the home and at our six-bed Hospice facility for terminally ill patients and those in need of palliative care. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for a minimum of one year after the death. Hospice and palliative care offer a sponsored care program to provide services regardless of patient’s ability to pay.

Reporting to the Admissions Director, this position is primarily responsible for (1) evaluation of pre-admission information as well as onsite discussions with the patient family to determine appropriateness of services and (2) completing the admission process.

**ESSENTIAL FUNCTIONS**:

- Explanation of Benefits/Insurance
- Explains benefits and hospice services to the patient and/or family member(s) as appropriate
- Explains to the patient and/or family the services covered by their insurance company and any out-of-pocket expense to them
- Explains the Patient’s Rights and Responsibilities
- Explains and secure signature on the consent and election forms
- Inquires regarding any advance directives in place, explains further regarding the right to make decisions about medical treatment and obtains signature
- Completes an assessment of the patient’s home safety
- Discusses, as appropriate, the preference of the patient regarding “Do Not Resuscitate” and completes the appropriate forms
- Completes all the documentation within 8 to 12 hours of the end of your shift and submit for processing and handoff.
- Clinical Initial Assessment
- Completes the initial assessment of the patient
- Assesses the patient’s current medication regime
- Completes a thorough pain assessment and follows-up for appropriate medications ordered and available in the home
- Consults with the patient’s physician, the pharmacist and other agencies as appropriate
- Communicates to the team manager, primary case manager, hospice physician, on-call team and family/pt the results of the initial assessment and any follow
- up issues and time frame
- Completes all the documentation within 8 to 12 hours of the end of your shift and submit for processing
- Initial Plan Of Care
- Completes and submits the completed chart by 10 am the following business day

**POSITION REQUIREMENTS**:

- Demonstrates the skills, knowledge and abilities necessary to be successful in the job as outlined in the position requirements
- Excellent interpersonal and communication skills
- Knowledge and understanding of the hospice philosophy and hospice criteria for admission and the admission process
- Knowledge of private insurance, Medicare and Medi-Cal regulations
- Knowledge of State and Federal regulations and Joint Commission

**EXPERIENCE AND EDUCATION**:

- Valid California registered nursing license
- California driver’s license and insurance
- RN experience within the last 2 years
- Hospice or home care experience preferred

**Job Types**: Full-time, Part-time

Pay: $59.00 - $62.00 per hour

Expected hours: 24 - 40 per week
