Id/dd Coordinator

**Position Overview**
The ID/DD Coordinator is responsible and accountable for the coordination and administration of services for clients and their caregivers under the direct supervision of the Location or Executive Director.

**Essential Job Functions**
- Provide initial evaluation to determine appropriateness and safety of client and environment.
- Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws
- Provide a written summary for individual clients and services based on needs, goals, and expected outcomes in conjunction with the ISP to Supports Coordinators/Administrative Entities on a monthly basis
- Participate in the development of Individual Support Plan
- Maintain patient confidentiality
- Demonstrate an understanding of client's medical status and updates self on clients/family status as needed or requested
- Maintain effective communication between staff, supports coordinators, and family
- Review progress notes from paraprofessionals to determine appropriate service delivery
- Educate paraprofessionals on individual client ISP expectations
- Perform on site evaluation of employee performance at 90 days and annually

**Requirements**:

- Bachelor’s degree in Psychology or other social science or related field
- Current CPR certification
- Proof of eligibility to work in the United States
- Current completion of child abuse clearance and Fingerprinting

**Preferences**
- Minimum of one year of experience with ID/DD preferred

**Physical Requirements**
- Must be able to speak, write, read and understand English
- Must be able to travel; company does not provide vehicles or transportation
- Frequent lifting, carrying, pushing and pulling of 25 pounds
- Occasional lifting, carrying, pushing and pulling of 50 pounds
- Prolonged walking, standing, bending, kneeling, reaching, twisting
- Must be able to sit and climb stairs
- Must have visual and hearing acuity
- Must have strong sense of smell and touch

**Environment**
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
