- Prior Home Health experience a must

**Field RN Case Manager Responsibilities**

Field RN Case Manager will manage all patients assigned to them and will perform all visits (admissions, routine visits, PRN visits, discharges once patient has met goals). Some Field RN Case Manager responsibilities:

- Active case Management of their patient caseload.
- Conduct visits where patients reside and perform assessments/routine SN visits
- Generate patients’ plan of care (POC) and oversee patients’ care per POC.
- Document all care and coordination provided for their patients
- Participate in the on-call rotation
- Manage their daily schedules with their patient caseload.
- ADON
- Advocate on behalf of their patients via proactive communication with their patients, families, and Physicians to ensure optimal outcomes for their patients
- Perform comprehensive assessments of their patients utilizing the OASIS data sets
- Be knowledgeable and have a good understanding of Home Health policies and procedures.

Pay: $75,000.00 per year
