**Salmon Hospice **is seeking a dedicated and compassionate Registered Nurse to coordinate and provide care to a caseload of hospice patients within a geographical area. Working both independently and as part of a team, the Hospice RN Case Manager ensures that quality care is delivered in a patient-centered, holistic, and sensitive manner to meet the needs of end-of-life patients and their families. Qualifications:

- Licensure: Registered Nurse Licensed to practice in Massachusetts
- Experience: Minimum 1 year clinical experience
- Hospice experience preferred
