Position Summary:

The Hospice Scheduling Coordinator position requires the ability to coordinate care within the hospice program. The Hospice Scheduling Coordinator should be able to coordinate/assign visiting disciplines to patients including but not limited to: nursing, social work, spiritual and bereavement counselors, certified registered nurse practitioners (CRNPs), hospice and homemaker aides, mental health and wound care nursing, physical therapy, occupational therapy, speech language pathology, and dietician as determined by the Interdisciplinary Group (IDG) and the individualized plan of care. Coordinates schedules between multiple providers and disciplines to assure optimal patient care, efficient utilization of services, and compliance with physician orders.

Required Experience:

  • Minimum of three (3) years previous work experience preferably hospice, home health or interdisciplinary team experience.
  • Demonstrates knowledge of Medicare, Medicaid, and other insurance regulations and terminology as it relates to Hospice.
  • Information system knowledge in the areas of electronic data entry and report generation.
  • Knowledge of Microsoft Outlook 365, Microsoft Teams, Microsoft Work/Excel, faxing, scanning, professional phone etiquette, and uploading documentation into EMR system.
  • Demonstrates high degree of commitment to customer service excellence, teamwork, organizational skills, detail orientation, flexibility, and ability to work with minimal supervision.
  • Demonstrates excellent verbal and written communication skills.
  • Ability to maintain attention and accuracy while attending multiple tasks simultaneously.
  • Ability to read and interpret documents, such as policy and procedures manuals, clinical documentation, and physician orders.
  • Ability to speak effectively before groups of customers or employees of the organization.
  • Compliant with accepted professional standards and practices.
  • Avid team player.
  • Consistently maintains a positive attitude which promotes team and optimal performance.

Responsibility for Hospice Scheduling Coordinator:

  • Coordinates scheduling of hospice patients with assistance of Clinical Director/Manager.
  • Performs scripted “Welcome Call” to all new patients.
  • Knowledge of hospice insurance benefit criteria and capable of explaining coverage to patients/families and callers.
  • Assists in the initial coordination of equipment, supply and medication delivery through patient/family/liaison contacts and schedule.
  • Responsible for maintaining the team master schedule and visit verification.
  • Participates in stand-up/stand-down meetings to manage visit changes related to exacerbations of symptoms, decline in patient status, and/or increased family support needs.
  • Creates and manages the Face-to-Face encounter schedule for hospice physicians or CRNPs.
  • Works with the Clinical Educator and Mentor to coordinate orientation schedules of all disciplines and participates in orientation of new staff.
  • Updates direct care staff schedules weekly and as needed.
  • Generates reports regarding visits and caseloads as scheduled.
  • Performs visit verification activities daily. Verifies visits provided to visits ordered, scheduled, and documented.
  • Notifies Clinical Director/Manager of discrepancies between visits ordered, scheduled, and documented.
  • Manages inpatient bed capacity throughout the day in collaboration with Inpatient Manager/Charge RN.
  • Coordinates with Clinical Director/Manager and Intake to schedule all patient admissions, including joint admissions with multiple IDG members.
  • Works under the direction of the Clinical Director/Manager to rearrange schedules in situations of call-offs, conflict, or high utilization of service.
  • Ensures prompt scheduling of emergent/as-needed visits as directed by the Clinical Director/Manager.
  • Proactively coordinates coverage of patient visits during planned staff illness or vacations.
  • Communicates with IDG members in respectful and professional manner
  • Notifies facilities and patients/families of unexpected schedule changes and follow-up plan.
  • Assures timely completion of individual staff schedules within established time parameters.
  • Accesses patient and provider schedules, authorizations, and physician orders to determine appropriate visit schedules under the direction of the Clinical Director/Manager.
  • Assures efficient use of staff by utilizing full-time staff prior to scheduling part-time/per diem staff.
  • Coordinates, assigns, and notifies appropriate on-call staff and management of Weekend/Holiday schedule.
  • Participates in staff meetings and education and other agency activities as requested.
  • Demonstrates an understanding of the hospice philosophy, QAPI, policies, procedures, and workflow processes.
  • Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and policies and procedures; maintains confidentiality of patient and employee records and information; has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to the Administrator and/or Quality Coordinator.
