We proudly lead advocacy efforts that rally our community to promote human rights, social justice, and economic opportunities for all. **_We are built upon the simple truth that people are experts in their own lives._** Our programs are spread across 25 locations and include over 1200 units of supportive housing in three boroughs, the Howie the Harp Advocacy Center, East Village Access—a PROS program, Peer-driven Crisis Respite Center, Adult Home Initiative and Assertive Community Treatment (ACT).
**Department Overview**
Community Access provides affordable housing and supportive services to individuals with psychiatric disabilities, histories of homelessness and substance use, families with histories of homelessness, and other low-income community members.
**Position**: Bilingual Entry Level Case Manager
**Work Location**:New York, NY
**Wage**: $26.21 / Hour

**Position Overview**
The **Bilingual Entry Level Case Manager** provides support services to assigned program participants, including assisting with defining personal, vocational, and educational goals. The **Bilingual Entry Level Case Manager **helps program participants using a person-centered model that supports individuals in their recovery, hopes and opportunities. As an **Bilingual Entry Level Case Manager**, you are responsible for providing recovery oriented support services and outreach to assigned program participants; developing and assisting in implementing service plans determined by goals of program participants, including reviewing service plans with program participants, identifying progress and revising plan as needed; producing and maintaining thorough, accurate and timely documentation, including charts and documentation of interactions with program participants, services provided, important information and/or events; assisting program participants in advocating for quality care received from external service providers, including educating providers on special issues, etc.

**Qualifications**
- Minimum of a high school diploma or equivalent (GED) Bachelor's degree, preferred.
- Must be fingerprinted and cleared by the Office of Mental Health (OMH).
- Microsoft Office experience
